Family Service Event Registration

This is where we should add details about the event. A new form will need to be created for each event and this section will also need to be updated too.

I acknowledge that images and/or recordings may be taken at the event by the event provider, organiser, sponsor, Childhood Cancer Association, or by other attendees and may be shared in print, digital, and social media communications.

By RSVP'ing to this event, I am indicating that I provide informed consent for images and/or recordings of me my family to be used in print, digital, and social media communications.
