CCA will provide the Family with temporary occupation at the Unit because their child is receiving medical treatment in Adelaide.

TERMS AND CONDITIONS:

Occupancy of the property
  1. CCA will allow the Family to occupy the Unit for such time or times as CCA may in its sole discretion permit.
  2. Nothing in this agreement is to be taken as implying in any way whatsoever that the relationship of landlord and tenant exists between CCA and the Family.
  3. The Family will be required to vacate the Unit immediately upon being given notice to do so by CCA whether verbally or in writing.
  4. The Family will not use the Unit otherwise than for the Permitted use.
  5. The Family will not do or permit any offensive, illegal, or dangerous activity at the Unit.
  6. The Family will keep the Unit secured and return all keys to CCA at the end of this Agreement
Inspections
  1. The Family agree to CCA undertaking regular property inspections to ensure the property is being adequately maintained. The Family will be given at least 7 days' notice by CCA prior to these inspections, either verbally or in writing.
  2. The Family are required to permit CCA, its servants and agents to enter the Unit at any time to inspect the state and condition of the Unit and its furnishings, fittings, and equipment.
Cleaning
  1. The Family is required to always keep the Unit clean and tidy and in good condition.
  2. The Family is required to leave the Unit clean, tidy and in the same condition (or as near as possible) as at the commencement of this Agreement.  If the Unit is not left clean and tidy the Family will be charged a cleaning fee charged of $250. 
Property Damage
  1. CCA will not be responsible for the loss, damage or theft of any items belonging to the Family while occupying the Unit.
  2. The Association will not be liable to the Family or any other party for any damage to person or property during the term of the Agreement.
  3. The Family must immediately report to CCA any break or breakdown of the furnishings, fittings, or equipment in the Unit.
  4. The Family occupying the unit will be requested to indemnify (pay back) the Association against any wilful damage caused to the Unit or any damage to or theft of any of the furnishings and equipment in the Unit by any occupant or guest of any occupant during the time the Family are in occupation of the Unit.
  5. No fixtures, fitting equipment or other items may be removed from the Unit without prior permission from CCA. 
Utilities

In consideration of CCA providing the Unit to the Family for the Permitted use the Family agrees: 

  1. To use the telephone provided in the Unit for local calls only. CCA will charge the Family for any other calls. In that event, an invoice will be provided monthly, and such payments must be paid within 14 days of receiving an account from CCA.  A final account for payment will be provided within two months of the conclusion of the period of occupancy.
  2. To ensure reasonable usage of utilities (e.g. gas and electricity). CCA may bill the tenant for excessive usage. 
Patient Assistance Transport/Travel Scheme (PATS)

Families who are eligible for PATS, may be required to complete regular paperwork. This will be communicated to the Family by CCA. It is the Family’s responsibility to ensure that the relevant PATS forms are completed on time as part of the condition of staying at the property. If you are finding this difficult, please contact the Family Service Team to discuss.

Other
  1. NO SMOKING /VAPING is permitted inside the Unit or on the property under any circumstances
  2. NO PETS are be permitted in the Unit or on any part of the premises
  3. Only immediate family or carers involved in the care of the child are permitted to stay at the Unit unless prior approval CCA has been given.
  4. Only one vehicle per Unit can be parked on the premises. No visitors are to park in the car park.

Any person(s) or persons not abiding by the conditions of this Agreement place this Agreement at risk and the Family may be asked to vacate the property either verbally or in writing.
