Case Management
The Case Management service is available to provide individualised support to families who require assistance to navigate supports available within the Childhood Cancer Association and the wider community, including benefits and entitlements, planning and advocacy.
Who can access the Case Management service?
Case management services are available to parents/caregivers and young people.
Who will I speak to?
Case management is provided by a qualified Case Manager from the Family Service team.
About the service
This service supports individuals and families to develop personalised goals and practical plans that address their unique needs and circumstances. Assistance may include:
- Guidance with completing and submitting applications for benefits and entitlements (e.g. Centrelink* and the Patient Assisted Travel Scheme (PATS)).
- Practical support to help access services.
- Connections to community organisations, including cultural, local, and specialist support services.
- Supporting referrals to Childhood Cancer Association services and other relevant childhood cancer support programs.
Assistance with navigating the local community, particularly for families who have recently relocated.
* Please note: This service can assist with the application process but cannot provide advice on eligibility for government benefits or entitlements. To determine what you may be entitled to receive, please contact Centrelink directly.
